PRE-ENROLLMENT TRAINING APPLICATION
Caregivers 2 Assist – Assisted Living Caregiver Training Program Thank you for your interest in our Caregiver Certification Program! To get started, all applicants must first register for the program. After registration, we’ll proceed with enrollment, which includes completing the necessary forms and pre-assessment tests. The total tuition for our 62-hour program is $500.00, which includes: 50 hours of distance learning 12 hours of hands-on, supervised skills training Required documents for certification: Valid U.S. photo ID Social Security card or “NO SS” Affidavit Food Handler Card Once you’ve completed the program, please contact us at 602-456-5437 or [email protected] so we can review your student portal and confirm your completion. The $65.00 state exam fee is included in the $500.00. Once you complete your training we’ll register you with D&S Diversified Technologies, LLP (Headmaster LLP) to schedule your exam. Important Information for the State Exam
Below are the required items needed to take the state exam. If you do not currently have all the items, you will need at least your ID/Driver's License and Social Security Card to enroll in our program.
